| ACTELIN 2 — Study of the Intracerebral Electrophysiological Signature in the Epileptic PatientAs part of the clinical management of certain severe, drug-resistant forms of epilepsy, it is necessary to implant electrodes in the brain to identify the epileptic network prior to potential neurosurgical resection of the source zones. This standardized procedure, known as Stereo-Electroencephalography (SEEG), was developed in France in the 1960s. The electrodes remain implanted for several days-typically two weeks-while awaiting spontaneous epileptic seizures. During this period, the patient remains hospitalized, and their electroencephalographic activity is recorded around the clock. Patients can be asked to perform cognitive tasks (such as tasks involving memory, attention, vision, language, and decision-making) while awake or asleep; the results can then be correlated with the intracerebral electroencephalographic activity recorded simultaneously as part of their clinical care. This approach makes it possible to capture the electrophysiological signatures of specific cognitive processes, thereby enabling researchers to: 1) create a brain map of electrophysiological signatures, 2) improve standard models of cognitive function, and 3) study the dysfunction of these processes in epilepsy. | Épilepsie | À vérifier | À vérifier | France | À vérifier |
| DIXI Medical Microdeep® Micro- Macro Depth electrodesThe main aim of this study is to evaluate the contribution of fast-ripples (FR) information on the neurosurgery management of patients with drug-refractory epilepsy investigated by Stereo-ElectroEncephaloGraphy (SEEG), as measured by freedom from disabling seizures one-year post-surgery. 220 patients (for whom 4-6 usual clinical macro-electrodes will be replaced by hybrid micro-macro electrodes and for whom fast-ripples will be assessed) distributed over 5 centres and 220 control patients(who will undergo an SEEG with the usual electrodes and for whom fast-ripples will not be assessed) distributed over 6 centres. | HYPOTHAMIG — Hypothalamus Connectivity in Chronic and Episodic MigraineThe purpose is to compare, using functional magnetic resonance imagery in resting-state, the connectivity of the hypothalamus in 2 groups of migraineurs. The first group is composed of chronic migraineurs, studied outside a migraine attack and is compared to gender- and age- matched episodic migraineurs with very few attacks per month and studied in the attack-free period. | Mesenchymal stem cellsMultiple sclerosis (MS) is a chronic inflammatory disease of the central nervous system (CNS), which ultimately leads to myelin damage and axonal loss. The disease is complex and multifactorial, but the key pathogenic event appears to be an uncontrolled response of components of the immune system (T and B lymphocytes) to myelin proteins. No definitive treatment is available for MS, however immunomodulatory and immunosuppressant drugs act as disease-modifying agents (DMDs). Unfortunately, the current treatments demonstrate partial efficacy in targeting the deleterious immune reactions. According to the present knowledge of the pathophysiology of MS, an ideal therapeutic strategy would be to modulate or suppress the aggressive immune process, to protect axons and neurons from degeneration, and to enhance repair and facilitate remyelination. A specific form of stem cells, called adult mesenchymal stem cells (MSCs), has shown remarkable ability to modulate the immune response. | Music Therapy in Alzheimer's DiseaseThe primary purpose of the protocol is to demonstrate that the use of music therapy may make it possible to discontinue at least one of neuroleptic treatment in patients with AD or related symptoms complicated by behavioral disorders such as agitation and aggressiveness evaluated using the Neuropsychiatric Inventory (NPI). | Two-dose M0-M2 vaccination scheduleImmunocompromised individuals, such as patients with multiple sclerosis (MS) or diseases of the neuromyelitis optica spectrum (NMOSD) treated with an anti-CD20 monoclonal antibody, present an increased risk of shingles. The immunogenicity - and therefore the clinical effectiveness - of the recombinant vaccine against the varicella zoster virus based on glycoprotein E, recently recommended according to two-dose schedule two months apart in immunocompromised patients aged ≥ 18 years, could be significantly decreased when administered one month before and one month after the infusion of anti-CD20.
